APPLICATION REQUIREMENTS

Just by using MyCollegeDentist.com to find a dentist and showing up on time for your appointment, you are eligible to apply for the SMILE PROUD Scholarships. One eligible student will be randomly selected for each of the Spring and Fall semesters**. The winners’ eligibility will be evaluated and verified to ensure honesty.

Students must visit the dentist once per semester to be eligible for that semester’s scholarship. 

 

**Scholarship will be awarded ONCE for the 2018 school year & will be given out on Jan 1, 2019.

**We will begin awarding scholarships for both Spring and Fall semesters in 2019.

 

THERE IS NO G.P.A. REQUIREMENT or ESSAY REQUIRED TO BE ELIGIBLE FOR THIS SCHOLARSHIP.

Just quickly complete the application requirements and form below after you visit your MCD dentist to officially apply!

 

Complete the following steps to be eligible to win :

    1. Be ACTIVELY ENROLLED at a college or university in the U.S. and be able to provide proof (i.e. transcript)
    2. Schedule an appointment with a dentist listed on MyCollegeDentist.com and arrive to your appointment on time *
    3. Schedule and attend at least ONE dental visit during the semester in which you’re applying for the scholarship.
    4. Post an honest and respectful review of your appointment on the MCD dentist profile
    5. Post a picture on either facebook and/or instagram of you smiling with your dentist, tag @mycollegedentist in the picture, and in the caption have at least #MCDSmileProud
    6. Complete the quick application info below
    7. Complete the Rafflecopter entry contest under the application form

  1. On the box below, click the drop-down tab ( ☰ ) in the top right corner.
  2. You will then be asked to sign in using either Facebook or email address, it does not matter which you chose.
  3. Finally, click the tab that says “Create / Login to Rafflecopter” to submit!

STUDENT NOTICES

* To be officially entered to win, you must log into Rafflecopter using the box above. We use Rafflecopter to help ensure that the winners of the SMILE PROUD scholarships are selected fairly. If you complete all other requirements but fail to register with Rafflecopter, you will NOT be entered into the drawing to win.

 

** All raffle scholarship applications will be reset on Jan. 1st and July 1st every year. It is highly advised that students visit the dentist at least twice a year for their routine cleaning and exam to help maintain the best possible oral health!

 

*** Being late to your dental appointments will make you ineligible for scholarships and not showing up for your appointment without proper notice may suspend your student scholarship eligibility until further notice. We realize how valuable everyones time is, so It is EXTREMELY IMPORTANT for ALL students to always arrive for their scheduled dental appointments on time.
